NYS Education Department has asked all school districts to send out this Digital Access Survey to determine the needs of students in terms of their access to internet services and devices. The importance of this information has been highlighted by the COVID-19 pandemic, and your response will help us with an accurate picture of the state of digital equity for each individual student. We would like you to submit one survey for every school-aged child in your home. We will be contacting parents and guardians, via mail or phone, who do not respond to this short 9 question digital survey. Please be assured that all responses are strictly confidential.
